To make the most of your multi-life cases, please fill out and then fax or email the prequalification form in the link above, along with a company census. The census should contain: employee marital status, salary, state of residence, and if possible, title.

Please follow up with a call to our office to schedule a time to discuss the case. We will do a complete analysis based on the pre-qualification form and census. A timeline is then generated to take you through each step: determining plan design and creating rate sheets, contacting the carriers to discuss the case, obtaining approval by the carrier and the employer, signing of the approval letter, planning for the education and communication of the benefit to employees, setting up the enrollment process and finally, submitting the case.
