Forms and Apps provides everything you need to make a sale: applications, outlines of coverage, marketing pieces and order forms.  If you need it, we either post it on our website or provide a direct link to the carrier website.

Online Quotes contains a link that will allow you to request a proposal from LifeStyle. We often use a web-based comparison quoting system when you request a multiple quote from our staff. We offer direct access to this system as a free service to our Premier Producers.  In partnership with VitalLTC, LifeStyle has the best on-line quoting and comparison system available.    Unlike some quoting systems, this one is created in contract with the carriers. This website provides comparisons and quotes that are ACCURATE and EASY to USE.
